Your First Visit

To expedite check-in and help us keep things running on time, please take a moment to read the following and fill out your paperwork before arriving.

Before your visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Usually, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays your dentist has sent you with, if applicable

  • A list of medications you are presently taking

  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims

IMPORTANT A parent or guardian must accompany all patients under 18 at the consultation visit.

Dental Insurance Information

At Leawood Commons Endodontics, we make every effort to provide you with quality care and the most convenient financial options.  In this endeavor, we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. They are well informed and up-to-date.

We have financing available through CareCredit – 0% interest up to 12 months finance plan – now easier than ever to qualify.

We also offer 10% Senior Citizens discounts for our patients age 65 and older.

Co-payments are due at the time of service. We accept cash, checks, debit, and all major credit cards.

Financial Policy

We are committed to delivering quality endodontic care at the most reasonable cost to our patients.   If you have dental insurance, please have your current insurance card available for us so that we may confirm your dental benefit coverage. We are more than happy to help you receive your dental benefits by filing your insurance claims for you.  We will provide you with an estimate for your co-payment amount and we will collect that estimated portion at the time of treatment. For your convenience we accept cash, checks, Visa, MasterCard, American Express, CareCredit and Discover. If needed, we happily offer 0% interest financing options through CareCredit. If you are needing to discuss your co-pay prior to any treatment, please let your assistant be aware of your needs and a member of our staff will do our best to provide you with that information.

Patient Billing

We will send you a monthly statement of any remaining balance. Most insurance companies will respond within four to six weeks. Please be advised that patients are responsible for payment of unpaid balances in the event that insurance does not cover the entirety of the procedures.
